Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that facilitates research into pragmatic trials. It collects and distributes clean trial data, ratings and evaluations using PRECIS-2. This allows for diverse meta-epidemiological analyses to evaluate the effects of treatment across trials with different levels of pragmatism.
Background
Pragmatic trials provide real-world evidence that can be used to make clinical decisions. However, the usage of the term "pragmatic" is not consistent and its definition and assessment requires clarification. The purpose of pragmatic trials is to guide clinical practice and policy decisions, rather than to prove the validity of a clinical or physiological hypothesis. A pragmatic trial should aim to be as close as possible to the real-world clinical practice which include the recruiting participants, setting, designing, delivery and execution of interventions, determination and analysis outcomes, and primary analysis. This is a major difference between explanatory trials, as defined by Schwartz & Lellouch1, which are designed to confirm a hypothesis in a more thorough manner.

In addition to these characteristics the pragmatic trial should also reduce the trial's procedures and data collection requirements to reduce costs. Furthermore pragmatic trials should strive to make their results as applicable to real-world clinical practice as is possible by making sure that their primary analysis is based on the intention-to-treat method (as described in CONSORT extensions for pragmatic trials).

Many RCTs which do not meet the requirements for pragmatism but have features that are contrary to pragmatism, have been published in journals of various types and incorrectly labeled as pragmatic. This could lead to false claims about pragmatism, and the use of the term should be standardized. The creation of the PRECIS-2 tool, which offers an objective standard for assessing pragmatic characteristics, is a good first step.
Methods
In a practical trial, the aim is to inform policy or clinical decisions by showing how an intervention could be incorporated into real-world routine care. This is distinct from explanation trials that test hypotheses regarding the causal-effect relationship in idealized situations. Therefore, pragmatic trials might have lower internal validity than explanatory trials, and could be more susceptible to bias in their design, conduct, and analysis. Despite these limitations, pragmatic trials can be a valuable source of information for decision-making in the context of healthcare.
The PRECIS-2 tool evaluates an RCT on 9 domains, ranging between 1 and 5 (very pragmatic). In this study, the recruit-ment organization, flexibility in delivery and follow-up domains scored high scores, but the primary outcome and the procedure for missing data fell below the limit of practicality. This suggests that it is possible to design a trial with high-quality pragmatic features, without compromising the quality of its outcomes.
It is, however, difficult to assess how pragmatic a particular trial is since the pragmatism score is not a binary quality; certain aspects of a study can be more pragmatic than others. The pragmatism of a trial can be affected by changes to the protocol or logistics during the trial. In addition 36% of the 89 pragmatic trials identified by Koppenaal and colleagues were placebo-controlled or conducted prior to licensing, and the majority were single-center. They are not close to the standard practice and can only be considered pragmatic if their sponsors accept that these trials aren't blinded.
A common aspect of pragmatic studies is that researchers try to make their findings more meaningful by studying subgroups of the trial sample. However, this often leads to unbalanced comparisons with a lower statistical power, which increases the chance of not or misinterpreting differences in the primary outcome. In the case of the pragmatic trials that were included in this meta-analysis this was a significant problem since the secondary outcomes were not adjusted for differences in the baseline covariates.
Furthermore, pragmatic studies can pose difficulties in the collection and interpretation of safety data. This is due to the fact that adverse events are typically self-reported and are susceptible to delays, errors or coding errors. It is therefore crucial to enhance the quality of outcomes ascertainment in these trials, and ideally by using national registries rather than relying on participants to report adverse events in the trial's database.
Results
While the definition of pragmatism does not mean that trials must be 100% pragmatic, there are benefits to including pragmatic components in clinical trials. These include:
Increasing sensitivity to real-world issues as well as reducing cost and size of the study as well as allowing trial results to be faster implemented into clinical practice (by including patients from routine care). However, pragmatic trials may also have drawbacks. For instance, the right type of heterogeneity could help the trial to apply its results to different patients and settings; however the wrong kind of heterogeneity can reduce assay sensitivity and therefore reduce the power of a study to detect minor treatment effects.
A number of studies have attempted to classify pragmatic trials with a variety of definitions and scoring systems. Schwartz and Lellouch1 have developed a framework that can differentiate between explanation studies that prove the physiological hypothesis or clinical hypothesis, and pragmatic studies that help inform the selection of appropriate treatments in real world clinical practice. The framework was comprised of nine domains evaluated on a scale of 1-5, with 1 being more informative and 5 was more practical. The domains were recruitment setting, setting, intervention delivery, flexible adherence, follow-up and primary analysis.
The initial PRECIS tool3 featured similar domains and an assessment scale ranging from 1 to 5. Koppenaal and colleagues10 created an adaptation of the assessment, called the Pragmascope that was simpler to use for systematic reviews. They discovered that pragmatic systematic reviews had higher average scores across all domains but lower scores in the primary analysis domain.
This difference in primary analysis domains can be explained by the way that most pragmatic trials analyze data. Certain explanatory trials however don't. The overall score was lower for pragmatic systematic reviews when the domains on the organization, flexibility of delivery and follow-up were combined.
It is crucial to keep in mind that a study that is pragmatic does not necessarily mean a low-quality study. In fact, there are increasing numbers of clinical trials which use the term 'pragmatic' either in their abstract or title (as defined by MEDLINE however it is neither sensitive nor precise). The use of these terms in abstracts and titles may suggest a greater awareness of the importance of pragmatism but it is unclear whether this is evident in the content of the articles.
Conclusions
In recent years, pragmatic trials are gaining popularity in research as the importance of real-world evidence is becoming increasingly acknowledged. They are clinical trials randomized that evaluate real-world alternatives to care instead of experimental treatments in development. They have patient populations that more closely mirror the patients who receive routine care, they use comparisons that are commonplace in practice (e.g. existing medications), and they depend on participants' self-reports of outcomes. This method has the potential to overcome the limitations of observational studies, such as the biases associated with reliance on volunteers and the lack of availability and coding variability in national registries.
Pragmatic trials also have advantages, including the ability to use existing data sources and a higher chance of detecting significant differences than traditional trials. However, these tests could be prone to limitations that undermine their reliability and generalizability. Participation rates in some trials may be lower than anticipated because of the healthy-volunteering effect, financial incentives, or competition from other research studies. Many pragmatic trials are also restricted by the necessity to recruit participants in a timely manner. Some pragmatic trials also lack controls to ensure that the observed differences aren't due to biases in the trial.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-described themselves as pragmatic and were published from 2022. The PRECIS-2 tool was used to evaluate the pragmatism of these trials. It covers domains such as eligibility criteria as well as recruitment flexibility as well as adherence to interventions and follow-up. They discovered that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or more) in at least one of these domains.
Trials that have a high pragmatism score tend to have higher eligibility criteria than traditional RCTs, which include very specific criteria that are unlikely to be used in the clinical environment, and they include populations from a wide variety of hospitals. The authors suggest that these traits can make the pragmatic trials more relevant and useful for everyday clinical practice, however they do not guarantee that a pragmatic trial is completely free of bias. In addition, the pragmatism that is present in trials is not a predetermined characteristic; a pragmatic trial that does not contain all the characteristics of an explanatory trial may yield valid and useful results.
