How do we begin Person-Centered Transition Planning?

There are many ways to conduct the Person-Centered Planning meeting that are as individualized as your students. It is a good idea to have the student create or help create their own meeting agenda. Below I have modeled some ideas as to how I would plan for, conduct, and evaluate a meeting.

Before the Meeting-Begin gathering information and support the student in making decisions about what will be presented, who will attend, when/ where the meeting will happen, processes, tools. Send invites, create a presentation, rules, and agenda. Gather materials (markers/paper, snacks, notetaker)
During the Meeting-Introductions, Discovery, Resources, Strategize, Conflict Resolution, Summarize, Next Steps, Thank Everyone
After the Meeting-Review, Evaluate, Monitor progress, Revise with team if needed, and meet again on the agreed upon date.
